Does laser have any advantages over the scalpel? Does laser have any advantages over the scalpel? I see the CO2 laser as a significant addition to the procedures and potential of plastic surgery. The laser permits corrective surgery to be performed where that would not have been possible in the past or only with visible scarring. In the case of a lower blepharoplasty, for example, the laser technique enables me to avoid scarring in many cases and generally to achieve a superior result compared with conventional methods. What preparatory and aftercare procedures are involved with this new method? We try to speed up the healing process. Patients are accordingly offered preparatory treatment at the Hasenöhrl Clinic with gentle peeling to cleanse the skin and activate cell regeneration. After surgery, soft laser therapy and lymph drainage are applied to quickly eliminate the swelling and burning feeling. As soon as the first stable epithelial layer has formed over the reduced area, i.e. after about 6-7 days, our beautician shows patients the best way to conceal the residual reddening. Patients are also given full information with regard to protection from the sun, and eye and skin care. Do you think there is such a thing as an absolute ideal for beauty? Every face is unique and cannot be standardized on the basis of certain fixed rules. One of your favorite subjects and specialties is the human nose. Why is that? The nose is not only the central point in the human face but also a very complex and difficult area in anatomical terms. Completely new situations often arise during the procedure, and the right decision has to be taken quickly, e.g. to remove some of the cartilage, to make do with one suture, etc. I simply find the human nose fascinating! Are plastic surgery and cosmetics in competition with each other? No, quite the contrary. The surgical intervention creates a new aesthetic basis, and cosmetics should be used to maintain the new aesthetic for many years to come. In Europe it is thought that 30 percent of all men now undergo plastic surgery at some point in their lives. Until not so long ago men were not involved in cosmetic surgery at all, but things have now very much changed. Today men are more aware of their bodies, too; they are fascinated by the sixpack abs look and above all want to remain attractive as they grow older. Many of my male patients are in senior management. That is all part of a trend in which good looks are seen as a key to a successful career. Yes, where there are concentrations of excess fat deposition in certain parts of the body - hips, thighs and buttocks in women, and the abdomen in men. The excess fat can be removed from such problem areas by suction, with the additional advantage that no fat returns to those places where suction has been applied. But that does not mean that you will never put on weight again. A lasting aesthetic result is only possible where patients are willing to take enough exercise and to change their eating habits. Is there still demand for face lifts? Yes, but the approach has changed. Today patients come for minor procedures anytime after their mid-thirties. In the medical profession we call this procedure "rejuvenation". The face is typically rejuvenated through a series of minor interventions performed every few years. The advantages are that hardly anyone notices that surgery has been involved, the complication rate is very low, and both women and men look more attractive whatever their age. Is cosmetic surgery medically necessary? It is not possible to make a clear distinction between medically indicated interventions and cosmetic surgery. An operation is medically necessary where some kind of disorder is involved. A woman with breasts she finds too small or too large is not suffering from a disorder. The operation may nevertheless be necessary because a disorder may result at the secondary level. Oversized breasts can cause damage to the vertebral column, which will necessitate complicated and expensive treatment at a later date. Breasts that are too small can cause psychological problems. In addition to cosmetic surgery, plastic and reconstructive surgery can also help patients regain their feeling of wellbeing and live a full life. That applies in particular to reconstruction after surgery to remove a tumor, unsightly scarring, cleft lip and palate, and serious accidents. Do you ever refuse to operate for cosmetic reasons? There are patients who come to me with completely unrealistic hopes and expectations. I advise such patients against surgery or I offer counseling so that they understand what procedures are possible and meaningful. There are also patients who want cosmetic surgery to compensate their problems. And I never operate when I see that the decision to go for surgery is not the patients' own decision but has been forced on them by someone else in their lives. |
