The classic face lift as a standard procedure no longer exists. The wide range of treatments and techniques available today offer the right therapy for every individual face. In many cases a better and more natural result is achieved with a simpler and less stressful procedure using the endoscope or with an additional injection treatment.
What happens to the face in the course of time?
The changes that we refer to as the aging process are basically related to gravitational pull. As the connective tissues of the face become less elastic and resilient, the eyebrows sink so that the frontal muscle has to actively raise the brow. That causes wrinkles to form across the forehead and makes the eyes look tired. In the zygomatic arch region there is a small fat pad that sinks in the course of time and undergoes a loss in volume. This leaves a hollow-eyed expression and also emphasizes the fold between the nose and the mouth. A similar fat pad in the cheek region sinks to the lower margin of the jaw and thus interrupts the youthful line of the jaw. Some of these changes can be veiled with the help of certain tricks, for example injecting the patient's own fat into the more pronounced nasolabial fold. But all these little procedures are artificial devices that ignore the true causes of aging phenomena, i.e. gravitational pull. The only way of achieving natural repositioning is operative lifting. The surgery should not cause any distortion or tension in the facial skin. A good face lift cannot be detected; patients look relaxed and youthful without showing signs of having had an operation.
What is the best answer?
What kind of treatment makes sense can be decided at the first exploratory consultation. Depending on the structure of the patient's skin, the basic choice is made between laser treatment, endoscopy or an open surgical procedure. The classical face lift comprises reduction of the skin in the neck and cheek regions and elevation of the underlying tissue. The skin is not used for attachment purposes, as that produces an unnatural tense expression. Working with the CO2 laser reduces swelling and hematomas to a minimum so that the obvious signs of surgery disappear after a week in the normal case. The scar runs from the temple along the inside of the small cartilage in front of the acoustic meatus to the earlobe and extends a short distance horizontally into the hairline. That makes the incision sound quite big, but ultimately all you can see is a fine line in the upper and very bottom section of the ear, where there is normally a crease in any case. After just 24 hours the dressing comes off. The sutures are removed on the 6th and 12th days.
